Girl Score is a hidden statistic that tracks Eric(a)'s progression towards femininity. Differing from Acceptance which indicates how much Eric(a) has willingly accepted being more feminine, Girl Score roughly tracks the amount of transformations that Eric(a) has accumulated, and how feminine they have become in body and mind regardless of whether they accept it or not.

Transformations that give Girl Score:
- Blond Hair Transformation (1 point)
- Breast Growth Transformation (1 point per stage; 7 points max currently)
- Bubble Butt Transformation (1 point)
- Corset Training Transformation (1 point when complete)
- Delicate Shoulders Transformation (1 point)
- Facial Transformation (1 point)
- Feminine Mannerisms Transformation (1 point per stage; 3 points max)
- Feminine Voice Transformation (1 point)
- Flawless Skin Transformation (1 point per stage; 2 points max)
- Girlish Arms Transformation (1 point)
- Hair Accessories Transformation (1 point)
- Height Reduction Transformation (1 point per stage; 2 points max)
- High Heels Transformation (1 point for either component; 1 point max)
- Hips Transformation (1 point)
- Penis Growth Transformation (-1 point)
- Penis Reduction Transformation (1 point per stage; 6 points max)
- Sensitive Nipples Transformation (1 point)
- Sexy Waist Transformation (1 point)
- Swimmer Legs Transformation (1 point)

Radical Transformations will also change Eric(a)'s body such that they temporarily have certain transformations and usually the corresponding Girl Score. The applicable transformations are:
- Cutie Magic Transformation (Pseudo-sixth stage Penis Reduction Transformation; no change to Girl Score)
- Succubus Shift (Sixth stage Penis Reduction Transformation)
Uses
Girl Score will affect dialogue in many parts of the game. There are however several places where Girl Score will also make a more concrete gameplay difference:
- Corset Training (5): Five Girl Score is required to complete the Corset Training Transformation.
- Marianne - Watanabe Inn (9): Nine Girl Score is required on the first day at Watanabe Inn if you undress in front of Marianne in order to avoid the Caught In The Onsen Game Over.
- Chad Summoning (17): Seventeen Girl Score is required to seduce Chad to get the Outsmarted escape from the Chad Summoning event.
