The Talisman is an item found in Prisoners of the Elder One. It is hidden inside the Mirror, accessible by going to the Nexus and using the Mirror, which has lost its reflective surface (the Hand Mirror shows the same behaviour and thus acts as a clue for this.) On its own, it does nothing, but when combined with the Glowing Ruby, it can be used in the pool in the Garden to safely traveled to the path to the Elder One's Hideout.
